Responsibilities:
Conduct initial and ongoing evaluations of residents' physical function and identify rehabilitation goals and objectives.
Develop and implement individualized treatment plans to achieve maximum functional improvement and independence.
Provide physical therapy treatments, including exercises, modalities, and other interventions as prescribed.
Collaborate with other healthcare professionals to ensure comprehensive care and achieve optimal outcomes.
Communicate effectively with residents, families, and caregivers to educate and inform them of treatment plans and progress.
Document all treatment sessions and progress accurately and thoroughly.
Participate in quality improvement initiatives to continuously improve resident care and outcomes.
