Overview
We are seeking a dedicated and compassionate Case Manager to join our team. The ideal candidate will be responsible for providing support and guidance to individuals facing various challenges, including behavioral health issues, addiction, and family dynamics. This role requires a strong commitment to intervention and a passion for helping others navigate complex systems such as social services, healthcare, and workforce development. This is a home-visiting program.
Duties
- Conduct assessments to identify clients' needs and develop personalized care plans.
- Provide crisis intervention and management to clients in high-stress situations.
- Collaborate with other professionals to coordinate services.
- Utilize motivational interviewing techniques to empower clients in their journey.
- Advocate for clients’ access to resources such as Section 8 housing, childcare services, and Medicare benefits.
- Engage in workforce development initiatives to assist clients in finding employment opportunities.
- Maintain accurate documentation of client interactions and progress in compliance with organizational policies.
Experience
- A background in social work or a related field is preferred.
- Home-visiting experience preferred.
- Experience in behavioral health settings or child welfare programs is highly desirable.
- Proven skills in crisis intervention and management are essential.
- Knowledge of community resources related to childcare, housing assistance, and workforce development will be advantageous.
Job Type: Full-time
Pay: $22.50 per hour
