Job Overview
We are seeking a dynamic and dedicated Community Health Worker Supervisor to lead and support community health initiatives within diverse populations. Responsibilities
- Supervise and mentor community health workers, providing guidance on best practices in client communication, motivational interviewing, and community outreach strategies
- Develop and implement community health programs tailored to the needs of underserved populations, including outreach on Medicare, nutrition, and public health law policies
- Coordinate health education sessions, public speaking engagements, and community events to promote wellness and preventive care
- Oversee data collection efforts using EMR/EHR systems to monitor program progress and evaluate outcomes
- Collaborate with healthcare providers, social service agencies, and community organizations to enhance service delivery and foster partnerships
- Ensure compliance with public health regulations and maintain up-to-date knowledge of public health law policies
- Conduct client assessments and support individuals with disabilities or chronic conditions through personalized care plans
- Review all documentation provided by community health workers
Experience
- Proven experience working with culturally diverse communities and underserved populations in a public health or community outreach setting
- Strong background in nursing, social work, teaching, or related fields with familiarity in community health program planning and development
- Demonstrated ability to lead teams effectively while maintaining excellent communication skills in bilingual settings preferred
- Knowledge of Medicare, Medicaid, nutrition programs, and basic life support certifications is highly desirable
- Proficiency in Microsoft Office applications for report writing, data analysis, and program documentation
- Experience working with individuals with disabilities or chronic illnesses using motivational interviewing techniques
- Familiarity with EMR/EHR systems for data collection and client record management
