American Traveler is seeking an experienced RN for a Clinical Transplant (Post-Transplant) position at a major academic medical center, requiring a minimum of 2 years of Med/Surg or ICU experience along with transplant experience.

Outpatient clinic-based setting within a large academic medical center (500+ bed facility)

Post-transplant patient population, including heart transplant recipients
-
Responsibilities include attending transplant clinics in person on site

Day shift, 8-hour shifts: 8:00 AM – 4:30 PM, Monday through Friday

Schedule is 3 days per week with flexible days (not fixed)

Epic EMR experience preferred
-
Active MD or Compact RN license currently in hand

Current BLS certification required

Minimum 2 years of recent, strong Med/Surg or ICU nursing experience required

Transplant experience required

Experience at a facility with 500+ beds required

Strong patient triage skills, including telephone triage for both simple and complex problems

Proficiency with computer data entry and legible documentation

SSN and DOB required for consideration

1 professional reference required for consideration

Coordinate care and clinically manage post-transplant recipients, including prescription refills, medication reconciliation, and chart review

Triage patient calls and manage clinical concerns by telephone

Collaborate with interdisciplinary transplant team including pharmacy, social work, and nutrition

Heart transplant experience is strongly preferred
