A Brief Overview
The Discharge Support Coordinator with the help of the Care Transition Team will maintain timely patient discharges by completing all the behind the scenes tasks needed to get a patient safely to his/her next site of care. (Ex: IRF, Skilled nursing, LTAC, and AR).
What You Will Do

Build and send referrals to Post- Acute facilities via HER.

Document within the EMR and sticky note of HER.

Proactively follows up with inpatient working referrals.

Roundtrip- for transportation needs.

Government regulation requirements- Completing entry of government required documents

Close out EHR referrals within 24 hours after discharge.

General Comprehension of CMS rules (Medicare) and familiarity with insurance policies.

Communication with team and post -acute facilities via phone or HER.

This position will maintain strong collaboration and teamwork throughout all assignments an interactions will departments.

Initiates and maintains a high level of customer service and positive employee relationships.

This position will require working rotating weekends, holidays and evening shifts as needed by department volume.

Ability to function effectively under department specified timelines and strong ability to effectively manage varying workloads.

Education

High School Diploma / GED (Required)
Work Experience
-
1+ years in a post-acute setting (Preferred) or
-
1+ years managing admissions in a post acute setting or referral process (Preferred)

Prior job experience with basic computer skills. (Required)
Knowledge, Skills, & Abilities

Knowledge of all levels of post-acute levels of care (Preferred proficiency)

Confident, motivated self-starter and demonstrate flexibility, initiative, patient focus and the ability to handle stress (Required proficiency)

Must exercise creativity and problem- solving skills with the ability to communicate effectively with persons of various backgrounds ( oral and written) (Required proficiency)

Preferred Qualifications/Position Details
Previous experience in admissions within a skilled nursing unit is strongly preferred. Knowledge of acute rehab is also preferred. This position will be supporting care transition staff with regulatory compliance for discharge planning and next site of care requirements. Schedule is Mon-Fri, 8:00am-4:30pm.
