The Registered Nurse (RN) will travel in the Tulsa and surrounding areas. There is no overnight travel. The RN will be responsible for performing accurate OASIS collection, new admissions, routine care visits, resumption of care, recertification, providing patient care & patient/caregiver education and completing coordination of client care. The Registered Nurse (RN) is the point of contact for all disciplines involved with providing care to patients and oversees the plan of care throughout the episode. The RN consults as needed with the physician and the office giving details about patient care. The RN will supervise care of LPN's and HHA's and be responsible for following all policies and procedures of the agency regarding service delivery, documentation and care coordination. A fleet vehicle is available.

- Strong assessment skills and clinical knowledge
- High sense of integrity including maintenance of confidential information
- Ability to work a flexible schedule to include some evenings and weekends when needed
- Ability to travel to patient homes in the local community
- Knowledge of EHR programs and computer skills
- Valid drivers license and auto liability insurance
- Current state license in good standing as a Registered Nurse (RN)
- 1 year of experience in a clinical care setting
- Experience in a home health environment or hospice preferred
- Passion for patient care.
