Certified Nursing Assistant (CNA)

Position Overview
Community Hospice is seeking a dedicated and compassionate Home Health Aide / Hospice Aide with an active Certified Nursing Assistant (CNA) credential listed on the State of Michigan Certified Nursing Assistant Registry. This position begins as PRN (as-needed) with the opportunity to transition to part-time or full-time status based on performance, availability, and organizational need. The Hospice Aide will deliver high-quality, person-centered care to patients and families in their homes throughout Berrien County, MI, working as an integral member of our interdisciplinary hospice care team to promote comfort, dignity, and quality of life.
Key Responsibilities
- Provide personal care including bathing, grooming, dressing, oral hygiene, and skin care for home health and hospice patients
- Assist patients with ambulation, positioning, transfers, and range-of-motion exercises per the care plan
- Observe and document patient condition, comfort levels, and changes; report significant changes promptly to the supervising RN or Hospice Team
- Support end-of-life comfort care, including pain and symptom management assistance as directed by the clinical team
- Provide emotional support and compassionate presence to patients and their families during the hospice journey
- Assist with medication reminders and prescribed therapeutic activities as outlined in the plan of care
- Perform light housekeeping, laundry, and meal preparation as related to patient care needs
- Maintain timely and accurate documentation of all care provided in accordance with agency policy
- Adhere to all infection control, safety, HIPAA, and agency compliance policies
- Travel to patient homes throughout Berrien County, MI.
Required Qualifications
- Active CNA certification listed on the State of Michigan Certified Nursing Assistant Registry
- Active CPR/BLS certification (must remain current throughout employment)
- Minimum 1 year of verifiable home health and in-home hospice care experience
- Valid Michigan Driver's License with a clean driving record
- Proof of current auto owners' insurance
- Clean criminal background check (required prior to start of employment)
- Reliable personal vehicle for travel throughout Berrien County service territory
- Ability to work a PRN schedule with flexibility; availability for evenings, weekends, and holidays as needed
- Ability to lift, position, and transfer patients safely using proper body mechanics
- Strong written and verbal communication skills
- Demonstrated empathy, professionalism, and sensitivity when working with patients at end of life and their families
Preferred Qualifications
- Hospice and Palliative Care Nursing Assistant (HPNA) certification or equivalent training
- Experience supporting patients and families through the bereavement process
- Familiarity with electronic health records (EHR) and point-of-care documentation
- Bilingual communication skills are a plus
Work Environment & Physical Requirements
Work is performed in patients' private homes and residential settings throughout Berrien County, MI. This role requires frequent standing, walking, bending, kneeling, and the physical ability to assist with patient mobility and personal care. You must be comfortable working independently in a wide variety of home environments and providing compassionate care during emotionally sensitive circumstances. Reliable personal transportation is essential.
Schedule & Employment Status
This position is offered on a PRN (as-needed) basis at the time of hire. Based on performance, demonstrated reliability, and the operational needs of Community Hospice, there is a clear pathway for this role to transition to a part-time or full-time position. PRN staff are expected to maintain minimum availability requirements as established by the agency.
