Seeking Full-Time Community Hospice Social Worker - Bronx, NY (LMSW/LCSW)
Responsibilities: Provides medical social services, following all hospice policies and procedures, to the patient and family in accordance with the interdisciplinary Plan of Care so that the function of each patient and the coping capacity of each family member will be maximized. Supports the Provider Relations and Quality Improvement activities of the organization.
Requirements:
- Master's Degree from a school of social work accredited by the Council on Social Work Education and the Education Department.
- Covid-19 vaccinated.
- Certified or licensed to practice social work by the state.
- At least one year of social work experience in a health care delivery system.
- Knowledge of computer operation, computer generated clinical records experience preferred.
- Ability to work with limited supervision.
- Strong interpersonal skills.
- Collaborative and team-oriented personality.
- Reliable motor vehicle required
- ***Bilingual Spanish***
Compensation:
- Compensation commensurate with credentialing and experience.
