All positions in the community require that we ask about your vaccination status. This includes recent TB test, flu, HEP B and COVID-19 vaccine status.

As a Licensed Practical Nurse (LPN) in our Skilled Nursing Facility, you will provide compassionate, high-quality nursing care to geriatric residents. You will assist in the planning, implementation, and evaluation of resident care and support the interdisciplinary team to ensure optimal health outcomes for elderly residents.

Deliver direct nursing care to geriatric residents under the supervision of an RN.

Administer medications and treatments in accordance with physician orders and facility policy.

Monitor and document residents' health status, including vital signs and any changes in condition.

Assist residents with activities of daily living (ADLs), including bathing, dressing, toileting, and feeding.

Maintain accurate and timely documentation in the Electronic Health Record (EHR).

Communicate effectively with residents, families, and members of the care team.

Ensure compliance with infection control policies, HIPAA regulations, and facility procedures.

Participate in shift handovers, audits, inservice training, and quality improvement programs.

Respond appropriately to medical emergencies or behavioral incidents.

Must be able to stand and walk for extended periods.

Requires lifting up to 50 lbs, and assisting with resident transfers and mobility.

May involve exposure to infectious materials, bloodborne pathogens, and emotional stress.

Must be available for various shifts including nights, weekends, and holidays as required.

Active and unencumbered LPN license in the state of employment (Required)

1+ years of experience in a Skilled Nursing Facility or geriatric care (Preferred)

Current BLS/CPR certification (Required)

Familiarity with EMR systems (e.g., PointClickCare preferred)
