You will be responsible for delivering personalized patient care, assisting with daily activities, and ensuring a safe and comfortable environment for all residents.
Duties
- Assist residents with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support using manual handling techniques and patient transfer methods such as Hoyer lifts.
- Monitor patients’ vital signs, observe changes in condition, and report findings accurately to healthcare teams while maintaining HIPAA compliance.
- Prepare nutritious meals tailored to individual dietary needs and assist with meal consumption to promote health and well-being.
- Support residents with medication administration under supervision, ensuring adherence to care plans and safety protocols.
- Engage residents in meaningful activities, provide companionship, and support cognitive functions through dementia and Alzheimer’s care strategies.
- Maintain cleanliness of resident spaces by performing light cleaning duties and ensuring infection control standards are met.
- Document patient observations, update care plans regularly, and communicate effectively with team members about residents’ progress or concerns.
Requirements
- Prior experience working in group homes, assisted living facilities, nursing homes, or home health environments is highly preferred.
- Knowledge of patient monitoring techniques including vital signs measurement, medication administration, and dementia or memory care practices.
- Ability to assist with ADLs using proper manual handling and patient transfer methods such as Hoyer lifts; experience with heavy lifting is advantageous.
- Certification in CPR and First Aid is required; additional certifications such as HIPAA training or hospice care experience are a plus.
- Strong communication skills for effective client interaction and team collaboration; experience working with individuals with autism or neurodevelopmental disorders is beneficial.
- Demonstrated ability to handle sensitive situations with compassion while maintaining safety standards in home & community care settings.
- Flexibility to work weekends on either part-time or full-time basis; reliable transportation for client personal transportation assistance is preferred.
