We are looking for a compassionate, clinically skilled, and highly organized Home Health Physician to provide comprehensive medical care to patients in their homes and other community-based settings. In this role, you will evaluate, diagnose, and manage acute and chronic medical conditions for individuals who may have difficulty accessing traditional office-based care due to age, disability, mobility limitations, or complex health needs. The Home Health Physician works closely with nurses, therapists, social workers, caregivers, and primary care or specialty providers to develop individualized treatment plans that promote safety, independence, comfort, and improved quality of life. This position requires strong clinical judgment, excellent communication skills, and the ability to make informed decisions in varied home environments with limited on-site resources. The physician will conduct in-home assessments, review medical histories, reconcile medications, order and interpret diagnostic tests, and coordinate follow-up care to ensure continuity across the healthcare continuum. A successful candidate will be comfortable building trusting relationships with patients and families, educating them on disease management and preventive care, and addressing goals of care with empathy and professionalism. The role also includes maintaining accurate and timely documentation, complying with regulatory and payer requirements, and supporting care transitions to reduce avoidable hospitalizations and emergency visits. Home Health Physicians must be adaptable, attentive to patient safety, and committed to interdisciplinary collaboration while delivering evidence-based, patient-centered care. Responsibilities
- Conduct comprehensive in-home medical evaluations, including physical examinations, health history reviews, and functional assessments.
- Diagnose and manage acute illnesses, chronic diseases, and complex medical conditions in accordance with evidence-based standards.
- Develop, implement, and regularly update individualized care plans in collaboration with patients, families, caregivers, and interdisciplinary team members.
- Order, review, and interpret laboratory tests, imaging, and other diagnostic studies, adjusting treatment plans as needed.
- Perform medication reconciliation, prescribe appropriate therapies, and monitor treatment effectiveness, side effects, and adherence.
- Educate patients and caregivers on disease management, preventive care, safety practices, and when to seek additional medical attention.
- Document all encounters, clinical findings, treatment decisions, and care coordination activities accurately and in a timely manner.
- Coordinate referrals, follow-up services, and transitions of care with hospitals, specialists, home health agencies, and community resources.
Requirements
- Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO) degree from an accredited institution.
- Current, unrestricted physician license in the applicable state and DEA registration in good standing.
- Board certification or board eligibility in Family Medicine, Internal Medicine, Geriatrics, or a related specialty.
- Clinical experience in home health, primary care, geriatrics, palliative care, or other community-based medical settings.
- Strong ability to assess and manage medically complex patients independently in nontraditional care environments.
- Excellent interpersonal, communication, and patient education skills with a compassionate, professional bedside manner.
- Proficiency in electronic health records, clinical documentation, and care coordination across multidisciplinary teams.
- Valid driver’s license and reliable transportation, with willingness to travel to patient homes within the assigned service area.
Pay: $85,000.00 - $150,000.00 per year
