Duties
- Assess patients’ physical conditions through comprehensive evaluations, including physiology and anatomy knowledge, to develop personalized care plans.
- Implement evidence-based therapy techniques such as trigger point therapy, orthopedics, neurology interventions, and geriatrics-focused treatments.
- Collaborate with multidisciplinary teams to coordinate discharge planning and ensure seamless patient transitions across care settings.
- Document all patient interactions accurately in electronic health records (EHR) systems in accordance with clinical documentation standards.
- Provide therapeutic interventions tailored for outpatient rehabilitation centers, inpatient hospital units, or home & community care environments.
- Educate patients and caregivers on physiotherapy exercises, mobility strategies, and injury prevention techniques.
- Monitor progress regularly and adjust treatment plans to optimize outcomes while adhering to medical terminology and clinical protocols.
Requirements
- Valid state licensure as a licensed Physical Therapist with practicing in a physical therapy role.
- Demonstrated experience in acute care, outpatient clinics, inpatient hospital settings, or home health environments.
- Strong knowledge of physiology, anatomy, medical terminology, and clinical documentation standards.
- Proven ability to work effectively with geriatric patients, pediatric populations, and individuals with neurological or orthopedic conditions.
- Experience managing care plans for diverse patient needs including outpatient rehabilitation, inpatient hospital stays, or home & community care services.
- Familiarity with electronic health records (EHR) or electronic medical records (EMR) systems for documentation and patient management.
- Excellent communication skills to collaborate with healthcare teams and educate patients on physiotherapy techniques.
- Ability to handle discharge planning processes efficiently while maintaining high standards of patient care.

Pay: $50.00 - $65.00 per hour
