We are seeking compassionate and skilled Registered Nurses (RNs) and Licensed Practical Nurses (LPNs) to join our growing home care team. Our nurses provide one-on-one skilled nursing care to patients in the comfort of their homes, helping them maintain their independence while receiving high-quality, individualized medical care.

Home Care Nursing Services Include
- One-on-one skilled nursing care
- Patient assessments and ongoing monitoring
- IV infusion therapy
- Wound care and dressing changes
- Medication administration and management
- Tube feeding (PEG/G-tube) management
- Catheter care and management
- Ostomy care
- Diabetic care and insulin administration
- Respiratory treatments
Schedule
- PRN / Per Diem
- Day, evening, overnight, and weekend shifts available
- Flexible scheduling
- Opportunities for full-time hours based on availability and patient needs
Responsibilities
- Provide skilled nursing care in patients' homes.
- Perform comprehensive nursing assessments.
- Administer medications and treatments as prescribed.
- Manage IV infusions and vascular access devices.
- Provide wound care using evidence-based practices.
- Perform tube feeding and enteral nutrition management.
- Monitor patient status and report changes promptly.
- Maintain accurate and timely clinical documentation.
- Promote patient safety, comfort, dignity, and independence.
Qualifications
- Active Registered Nurse (RN) or Licensed Practical Nurse (LPN) license in the State of South Carolina.
- Current CPR certification
- Home care, private duty, hospice, rehabilitation, long-term care, or acute care experience preferred.
- Experience with IV therapy, wound care, tube feeding, tracheostomy care, and medication management is preferred.
- Reliable transportation and ability to travel within the Savannah area
