Summary
The MDS Lead (RN) is responsible for managing the timely completion of accurate assessments and interdisciplinary care plans that meet Federal and State guidelines. The MDS Lead (RN) identifies resident acuity and needs, helping to determine specific care needs and communicating needs / expectations to families and responsible parties.
Roles and Responsibilities
- Oversee/complete assessments, Minimum Data Set (MDS) and care plans for residents.
- Monitor completion of MDSs by other disciplines within timeframes prescribed by regulatory guidelines.
- Manage accurate, timely completion of the MDS information for residents.
- Initiate care plans and supporting activities that will result in best possible outcome for residents.
- Advises Director of Nursing of persistent issues related to non-compliant documentation on residents.
- Promote and participate in ongoing education, inside and outside the facility, to maintain and enhance knowledge of current / new regulations.
- Maintain current knowledge of MDS assessment process.
- Schedule meeting with the Rehab Director as needed in a manner that accurately captures the RUG (Resource Utilization Group) category.
- Provide a schedule assessment data as needed to the interdisciplinary team in order to facilitate the timely completion of MDS sections for each discipline.
- Oversee the completion of MDS sections according to facility assignments.
- Communicate to members of the IDT as needed based on timeliness of completion.
- Manage timely transmittal of PPS and Quarterly Assessments.
- Manage timely transmission of comprehensive assessments.
- Provide confirmation for the transmission file and review the printed initial and final validation report.
- Facilitate the correction of errors immediately and retransmit the assessment.
- Facilitate the completion and updating of resident care plans to reflect the residents’ most current needs.
- Identify residents in need of a Significant Change in Status.
- Lead facility staff education as it relates to the MDS process, and the coordination of obtaining accurate documentation from direct care staff.
- Compile data reports based on MDS data as requested by the facility Administrator and/or Director of Nursing.
- Update information for the Certified Nursing Assistant (CNA) care plans.
- Utilize the facility’s software program and computer system to comply with federal requirements for completion and encoding.

Education/Experience
· High School Diploma or GED
· A graduate of an accredited school of nursing.
· Minimum one (1) year experience in licensed nurse role.
Certificates and Licenses
· Registered Nurse
· Must have a minimum of one (1) year registered nurse experience.
· Must currently hold MDS Certification, OR have a minimum of one (1) year structured MDS training experience.
Job Type: Full-time
Pay: $55.00 - $60.00 per hour
