Job Summary
We are seeking a dynamic and experienced Director of Nursing - RN Home Health to lead our home health nursing services with passion and precision. Duties
- Provide comprehensive leadership and supervision for all nursing staff within the home health agency, fostering a positive and collaborative work environment.
- Develop, review, and implement individualized care plans for patients, ensuring alignment with best practices and regulatory requirements.
- Oversee documentation review, medical records management, and compliance with HIPAA regulations to protect patient confidentiality.
- Ensure adherence to Medicare, long-term care regulations, and other federal/state guidelines governing home health services.
- Manage staffing schedules, shift management, and resource allocation to maintain optimal patient coverage and staff productivity.
- Monitor clinical outcomes by analyzing data from EMR (Electronic Medical Records) systems and EHR (Electronic Health Records), driving continuous quality improvement initiatives.
- Lead regulatory audits and inspections, preparing necessary documentation to demonstrate compliance with all licensing standards.
- Collaborate with interdisciplinary teams including physicians, therapists, social workers, and administrative personnel to coordinate seamless patient care.
- Promote professional development through ongoing training in areas such as medical terminology, anatomy, physiology, memory care, hospice care, assisted living experience, nursing home experience, and acute care practices.
- Maintain a strong focus on patient safety by reviewing clinical documentation regularly and ensuring high standards of care are consistently met.
Experience
- Proven nursing leadership experience within home health agencies or similar settings such as assisted living facilities or long-term care environments.
- Extensive background in managing care teams in diverse settings including hospice care, nursing homes, or acute care units.
- Demonstrated expertise in supervising nursing staff with a focus on nurse management and administrative clinical supervision.
- Familiarity with EMR/EHR systems for documentation review and data analysis; proficiency in managing medical records securely.
- Strong knowledge of long-term care regulations, Medicare requirements, HIPAA compliance, and regulatory standards governing home health services.
- Experience coordinating multidisciplinary teams to develop effective care plans tailored to individual resident needs.
- Ability to lead quality improvement initiatives based on clinical data analysis and regulatory feedback.
- Excellent communication skills to effectively collaborate with healthcare professionals while maintaining a focus on resident-centered care.
- Management experience that includes shift management, resource planning, supervising staff performance, and fostering a culture of excellence in patient care.
