Key Responsibilities:
- Evaluate patients’ physical conditions, limitations, and rehabilitation needs
- Develop and implement individualized treatment plans
- Provide therapy in accordance with the physician’s Plan of Care
- Supervise and coordinate care with PTAs and HHAs
- Maintain accurate documentation and progress notes
- Educate patients and caregivers on exercises and equipment use
- Assess home environments and recommend modifications
- Collaborate with nurses, social workers, and other healthcare providers
- Submit visit notes on time while maintaining HIPAA compliance
Qualifications:
- Licensed Physical Therapist (PT)
- Minimum 1 year of PT experience preferred
- OASIS and WellSky (Kinser) experience preferred (training provided)
- Current CPR certification
- Physical exam (within last 12 months)
- TB test (within last 12 months)
- Valid driver’s license
- Must attend orientation at our Taunton, MA office
