Working Nurses Homecare is seeking a compassionate and experienced Speech-Language Pathologist (SLP) to join our growing home health team. This is a field-based position providing skilled therapy services to patients in the comfort of their homes.
As an SLP, you will evaluate and treat patients experiencing speech, language, communication, cognitive, and swallowing disorders while collaborating with physicians, caregivers, and members of the interdisciplinary care team.
Key Responsibilities
- Conduct comprehensive speech, language, cognitive, communication, and swallowing evaluations.
- Develop and implement individualized plans of care based on patient needs and physician orders.
- Provide skilled treatment to improve communication, cognitive-linguistic function, and swallowing abilities.
- Educate patients, families, and caregivers on treatment plans, techniques, and safety precautions.
- Monitor patient progress and modify treatment plans as clinically appropriate.
- Communicate and collaborate with physicians, nurses, therapists, and other members of the interdisciplinary team.
- Complete accurate and timely clinical documentation in accordance with company, Medicare, and regulatory requirements.
- Participate in care coordination and discharge planning as needed.
- Maintain patient confidentiality and adhere to HIPAA requirements.
Qualifications
- Master’s degree in Speech-Language Pathology from an accredited program.
- Current and unrestricted Florida Speech-Language Pathologist license.
- Certificate of Clinical Competence (CCC-SLP) preferred.
- Current CPR/BLS certification.
- Home health experience preferred.
- Experience working with adult and/or geriatric patients preferred.
- Strong clinical assessment, communication, and documentation skills.
- Reliable transportation and willingness to travel to patients’ homes within the assigned service area.
- Ability to work independently while effectively collaborating with the care team.
