Hospice Nurse Practitioner (NP) - Clinical Support
New Jersey (In-Office) | Monday-Friday, 8:30 AM - 5:00 PM + rotating on-call | Full-Time
Position Summary
The Hospice Nurse Practitioner serves as the primary advanced practice provider within the Moments Hospice Clinical Support Center. Based in our New Jersey office, this position provides real-time clinical support to hospice nurses, patients, families, facilities, and physicians across multiple states.
The Nurse Practitioner is responsible for managing urgent symptom management concerns, providing clinical guidance to field clinicians, reviewing and initiating medication orders, responding to escalated patient and family concerns, assisting with complex clinical decision-making, and completing Medicare-required Face-to-Face (F2F) encounters as needed.
Working closely with Medical Directors, hospice nurses, pharmacists, and interdisciplinary teams, the Nurse Practitioner helps ensure patients receive timely, compassionate, and evidence-based hospice care while serving as a trusted clinical resource throughout the organization.
Schedule
- Monday-Friday, 8:30 AM - 5:00 PM (based in the New Jersey office)
- Participation in a rotating on-call schedule (phone-based; frequency shared during interview)
- Full-time, 40 hours/week
Essential Responsibilities
Clinical Support
- Serve as the primary Nurse Practitioner supporting the Clinical Support Center during assigned business hours.
- Provide real-time clinical consultation to hospice nurses across multiple states.
- Assist field clinicians with complex clinical questions and patient care decisions.
- Collaborate with Medical Directors regarding complex patient situations.
Urgent Clinical Response
- Manage incoming urgent clinical calls from nurses, patients, families, caregivers, and facilities.
- Assess and provide recommendations for pain, dyspnea, anxiety, agitation, nausea, terminal secretions, delirium, and other end-of-life symptoms.
- Provide provider-level support for complex symptom management.
- Assist with difficult patient, family, and facility concerns requiring advanced clinical judgment.
Medication Management
- Review and initiate medication orders.
- Authorize medication refills.
- Modify medication regimens based on patient condition and goals of care.
- Collaborate with pharmacies to ensure timely medication access.
- Review medication appropriateness, effectiveness, and safety.
Face-to-Face Visits
- Complete Medicare-required Face-to-Face (F2F) encounters as needed.
- Evaluate continued hospice eligibility.
- Document findings in accordance with CMS regulations.
- Provide recommendations to the Medical Director regarding hospice recertification.
Clinical Collaboration
- Serve as a clinical resource for nurses, admissions staff, and interdisciplinary team members.
- Support timely resolution of urgent clinical questions.
- Participate in interdisciplinary meetings as appropriate.
- Promote compliance with hospice regulations and organizational policies.
Quality & Documentation
- Document patient interactions accurately and timely in Homecare Homebase (HCHB).
- Ensure compliance with Medicare Conditions of Participation, state regulations, and organizational standards.
- Participate in quality improvement initiatives and ongoing clinical education.
Qualifications
Required:
- Active, unrestricted New Jersey APRN/Nurse Practitioner license in good standing
- National certification as a Nurse Practitioner (AANP or ANCC) - FNP, AGNP, or AGACNP
- Current DEA registration and prescriptive authority
- Graduate of an accredited Nurse Practitioner program (MSN or DNP)
- Experience in hospice, palliative care, or a related clinical setting
- Familiarity with Medicare Face-to-Face (F2F) encounters and hospice eligibility criteria
- Strong symptom-management and end-of-life clinical judgment
- Comfortable providing telephone/telehealth clinical guidance and navigating an EMR while on calls
Preferred:
- Homecare Homebase (HCHB) experience
- Compact/multi-state RN background and willingness to obtain additional state NP licensure as needed
- 2+ years in hospice, palliative care, oncology, acute care, or ER
- Prior experience in a clinical support center, triage, or provider on-call role
Licensure Note (Multi-State Support)
This role supports clinicians and patients across multiple states. Providing orders and completing Face-to-Face encounters generally require licensure in the patient's state. The NP must hold an active New Jersey license and be willing to obtain additional state NP licenses as needed; Moments Hospice will support the licensure process. Out-of-state consultative guidance is provided within applicable scope-of-practice and state regulations.
