Overview
Supportive Care Specialists is seeking a compassionate and experienced Nurse Practitioner (NP) to join our growing Colorado team, providing supportive care across hospital, LTACH, and post-acute settings.
This role goes beyond traditional supportive care by emphasizing Transitions of Care, interdisciplinary collaboration, and proactive discharge planning. You'll work closely with physicians, case management teams, and hospital leadership to improve patient outcomes, reduce barriers to discharge, and support safe, timely transitions throughout the continuum of care.

Key Responsibilities
- Conduct daily patient rounding in hospital and LTACH settings.
- Perform comprehensive patient assessments and develop individualized care plans.
- Collaborate with attending physicians, hospitalists, case managers, nurses, and interdisciplinary teams.
- Support transitions of care by identifying and addressing barriers to timely discharge.
- Participate in discharge planning and interdisciplinary care discussions.
- Provide supportive care consultations and symptom management for patients with complex medical conditions.
- Communicate effectively with patients, families, and healthcare teams regarding care plans and treatment goals.
- Maintain timely, accurate, and compliant clinical documentation.
- Foster strong relationships with hospital staff and contribute to a collaborative care environment.
Qualifications
Required
- Active Colorado Nurse Practitioner (NP) license, or the ability to obtain licensure prior to start.
- Minimum of two (2) years of inpatient hospital experience.
- Strong clinical assessment, communication, and critical thinking skills.
- Ability to work collaboratively within an interdisciplinary healthcare team.
Preferred
- Experience in hospital medicine, internal medicine, geriatrics, rehabilitation, LTACH, or post-acute care.
- Experience with discharge planning, care coordination, or transitions of care.
- Experience in supportive care or palliative care is a plus.
