TRAVEL RN CASE MANAGER – WEST HOLLYWOOD, CA
- Location: West Hollywood, CA 90048
- Position: Travel RN – Case Manager
- Contract: 13 Weeks
- Schedule: 5×8 Days | 8:00 AM–5:00 PM
- Weekend: Every Other Weekend
- Start Date: September 14, 2026
- Weekly Pay: $2400 - $2800 (DOE)
Job Duties:
- Coordinate patient care and transitions throughout the hospital stay.
- Lead discharge planning and coordinate post-acute services.
- Perform utilization review and evaluate medical necessity.
- Develop individualized plans of care based on patient needs.
- Coordinate DME, transportation, and community resources.
- Collaborate with physicians, nurses, social workers, and interdisciplinary teams.
- Maintain accurate documentation using Epic.
- Support compliance with CMS and other healthcare regulations.
Requirements:
- 3+ years of acute-care Case Management experience
- Active California RN License in hand
- BLS certification
- Strong discharge planning & utilization review experience
- Epic experience preferred
- Flu vaccination required — no exemptions
