Responsibilities
- Assist clients with activities of daily living (ADLs), including personal hygiene, grooming, dressing, and mobility support using appropriate patient transfer methods such as Hoyer lifts or manual handling techniques.
- Monitor clients’ health by taking vital signs, observing patient conditions, and reporting changes promptly to healthcare teams.
- Support medication administration following HIPAA guidelines and care plans to ensure safety and compliance.
- Prepare nutritious meals tailored to individual dietary needs and assist with meal consumption when necessary.
- Provide transportation assistance for appointments, errands, or community outings to promote social engagement and independence.
- Maintain a clean, safe environment by performing light cleaning duties and ensuring proper infection control practices are followed.
- Document client progress accurately in care plans, communicate effectively with team members, and ensure compliance with all home & community care standards.
Requirements
- Prior experience working with individuals with disabilities, developmental or intellectual disabilities, or neurodevelopmental disorders is highly preferred.
- Knowledge of group home settings, assisted living environments, nursing homes, or hospice care is advantageous.
- Certification in CPR and First Aid is required; ongoing training in patient observation, dementia care (including Alzheimer’s and memory care), and medication administration is a plus.
- Ability to lift heavy objects safely and perform manual handling tasks such as patient transfers using equipment like Hoyer lifts or patient transfer methods.
- Strong communication skills for effectively engaging with clients, families, and healthcare professionals while maintaining confidentiality under HIPAA regulations.
- Flexibility to work various shifts including evenings, weekends, or holidays as needed in a dynamic caregiving environment.
