Location & Schedule
- Location: Hamden, CT & surrounding New Haven County
- Schedule: Full-Time / Part-Time (Flexible Mon–Fri and weekend options)
Your Impact
- Direct Patient Care: Perform health assessments, track vital signs, administer medications, and manage wound care directly in patients' homes.
- Patient Advocacy: Execute personalized care plans under RN guidance and serve as a trusted partner for your patients.
- Empowerment: Educate patients and families to foster independence and improve quality of life.
- Collaboration: Partner with physicians, RNs, and therapists to ensure seamless, top-tier care.
What You Need to Succeed
- Active Connecticut LPN License.
- Minimum 6 months of clinical nursing experience (SNF, LTC, Med-Surg, Rehab, or Hospital).
- Valid driver’s license, reliable personal vehicle, and active auto insurance.
- Strong communication, documentation, and time management skills.
- Bilingual (Spanish/English) or prior Home Health experience is a plus!

Pay: $27.00 - $30.00 per hour
