Position: Behavioral Respite Intake Coordinator
Pay Range: $26.00-$30.00 PER HR
Reporting To: Behavioral Respite Program Manager

POSITION SUMMARY:
The Behavioral Respite Intake Coordinator serves as the primary point of contact for incoming behavioral respite referrals and plays a vital role in creating a positive, seamless experience for consumers, families, and referral partners. This position is responsible for managing the intake and enrollment process from referral through admission, ensuring accurate collection of consumer information, medical histories, funding source documentation, and service records.
The Intake Coordinator coordinates closely with families, referral sources, clinical teams, and operational staff to facilitate timely admissions, maintain accurate records, and support efficient service delivery. Success in this role requires strong organizational skills, attention to detail, excellent communication, and the ability to manage multiple priorities in a fast-paced environment.

1. High school diploma or equivalent required.
2. Associate’s or Bachelor’s degree in Healthcare Administration, Business Administration, Health Information Management, Social Services, or a related field preferred.
3. Minimum of one (1) year of intake, admissions, care coordination, or data entry experience in a healthcare, behavioral health, social services, or related setting.
4. Experience working with Electronic Medical Record (EMR) systems preferred.

Intake, Enrollment & Records Management
1. Serve as the first point of contact for consumers, families, and referral sources regarding behavioral respite services.
2. Receive, review, and process incoming referrals through electronic systems, fax, phone, and other communication channels.
3. Gather and verify required referral documentation, medical histories, funding source information, and records related to current services.
4. Manage the consumer enrollment process from referral through admission, ensuring all required documentation is completed accurately and timely.
5. Enter and maintain consumer information within the Electronic Medical Record (EMR) system.
6. Maintain accurate, complete, and up-to-date consumer files and records.
7. Track and maintain referral, intake, and admission statistics and reports.
8. Ensure compliance with organizational policies, documentation standards, and confidentiality requirements.
Communication & Care Coordination
1. Communicate with referral sources, funding sources, families, and internal staff to obtain and verify required information.
2. Collaborate with clinical leadership, managers, and staffing coordinators to facilitate timely admissions and service initiation.
3. Provide regular updates to leadership regarding referral status, enrollment progress, and barriers to admission.
4. Maintain consistent communication with all stakeholders throughout the intake, consent, and service delivery process.
5. Coordinate and schedule intake meetings, assessments, and initial visits based on consumer and family availability.
6. Ensure smooth transitions and effective communication between intake, clinical, and operational teams.
Process Improvement & Administrative Support
1. Assist with the development, implementation, and continuous improvement of intake workflows, standard operating procedures, and tracking systems.
2. Identify opportunities to improve efficiency, accuracy, and the overall client experience.
3. Provide administrative support related to intake operations and departmental initiatives.
4. Perform other duties and special projects as assigned.

PHYSICAL REQUIREMENTS:
· Stand, sit, talk, hear, reach, stoop, kneel and use of hands and fingers to operate computer, telephone, and keyboard on a frequent basis (Up to 75% of the time).
· Close vision requirements due to computer work.
· Light to moderate lifting may be required (up to 25lbs).
