RN – Hospice Home Health Weekend-Only Contract
Location: Racine/Kenosha Territory, WI
Territory: Racine, Kenosha, and Mt. Pleasant, WI – RN will travel to patients’ homes throughout the assigned territory. May occasionally assist with nearby coverage if needed, generally within one hour of the home territory.
Schedule: Saturday & Sunday | 8:00 AM – 8:00 PM | 24 Hours/Week

We are hiring an experienced Registered Nurse – Hospice Home Health for a 10-week contract assignment servicing the Racine/Kenosha territory, including Racine, Kenosha, and Mt. Pleasant. This is a home hospice position, so the RN will travel directly to patients’ homes within the assigned territory.
Responsibilities
- Provide compassionate hospice nursing care directly in patients’ homes.
- Travel throughout the assigned Racine, Kenosha, and Mt. Pleasant territory to provide patient care.
- Care for patients with failure to thrive, advanced cancers, end-stage COPD, terminal illnesses, and other end-stage disease processes.
- Perform focused assessments and use critical thinking to identify and manage patient needs.
- Conduct PRN visits for symptom management, falls, and death visits as needed.
- Perform Foley catheter exchanges and troubleshoot feeding tubes.
- Provide wound care and post-fall assessments.
- Administer narcotic pain medications and monitor for potential medication diversion.
- Communicate with medical examiners/coroners during death visits when required.
- Document patient care using Epic Rover.
- Collaborate with the Lead RN and hospice team to ensure timely and appropriate patient care.
- May occasionally assist with coverage slightly outside the assigned territory when needed, generally within one hour of the home territory.
Requirements
- Minimum 2 years of Home Health or Home Hospice experience
- Active Wisconsin RN License.
- BLS Certification
- Active Driver’s License.
- Own reliable vehicle with valid auto insurance.
