Position Overview:
We are seeking a full-time Family Nurse Practitioner to serve as a float provider across multiple clinic location. This role includes a structured weekly schedule with designated days at specific offices, along with occasional coverage at additional locations as needed.
Working Hours:
Monday through Friday, 8:30 AM – 5:00 PM
No weekends or evening shifts required.
Job Responsibilities:
- Provide comprehensive primary care services across the lifespan, including health assessments, diagnosis, and management of acute and chronic conditions.
- Float between assigned clinic locations on a set weekly schedule, with occasional adjustments to support coverage needs.
- Order, interpret, and follow up on diagnostic tests; prescribe medications and treatments in accordance with scope of practice.
- Develop and implement evidence-based treatment and preventive care plans, including immunizations, screenings, and patient education.
- Collaborate with physicians and interdisciplinary team members to ensure high-quality, patient-centered care across all sites.
- Document patient encounters accurately in the electronic health record while maintaining compliance with regulatory and organizational standards.
- Promote health literacy and support patients and families in managing their health and wellness.

Pay: From $115,000.00 per year
