Job Summary
We are seeking a dedicated GAFC Direct Care Staff member to provide in‑home assistance to elders and adults with disabilities. The Direct Care Staff supports members with ADLs and IADLs, medication reminders, and monthly documentation in accordance with the GAFC Plan of Care. This role is essential in helping members maintain independence and safety in their homes.
Responsibilities
- Provide personal care and assistance with ADLs/IADLs
- Issue medication reminders and assist with prescription refills
- Complete visit logs and submit monthly documentation
- Monitor and report changes in member condition
- Maintain professional boundaries
Experience
- HHA certification preferred
- 1+ year experience with elder care or disability services
- Strong communication and documentation skills
- Reliable transportation preferred
