Responsibilities:
- Conduct telephonic and occasional in-home assessments of members’ medical, psychosocial, physical, and spiritual needs.
- Develop, implement, and manage personalized care plans in collaboration with PCPs, caregivers, and interdisciplinary care teams.
- Leverage benefit structures and community resources to address members' needs while promoting preventive health education.
- Complete and review initial and follow-up assessments to create and update person-centered service plans (PCSPs).
- Monitor changes in members' conditions, revising care plans and communicating updates to the care team.
- Present cases at interdisciplinary meetings and adhere to CMS/DOH guidelines to maintain care quality standards.
Qualifications:
- RN - Bachelor’s degree (BSN) preferred.
- Experience Minimum three to five years nursing experience in long term care, managed care, home care, geriatrics or hospice.
- Prior experience in an interdisciplinary service delivery environment preferred.
- Working knowledge of Medicare and Medicaid regulations.
- Prior experience in an interdisciplinary service delivery environment preferred.
- Specialized Knowledge and Skills, License/Certification.
- Current RN New York State license; CCM preferred.
