Position Summary:
The Registered Nurse (RN) partners with the Director of Nursing to oversee the care of the individuals ensuring the best practice and care of the individuals served. This role fosters a "One Team" approach, ensuring collaboration among medical, clinical, and operations teams to achieve the highest quality of care and support. Key responsibilities include travelling to homes, providing the highest quality of care to include medication management, medical treatment needs, education, and providing support to the Direct Service Providers.
We are willing to consider an RN or LPN for this position. Compensation is based on role. This Nurse will work in the field covering our group homes in Ypsilanti,, Carlton, Southfield, Wolverine Lake areas.

Primary Responsibilities:
- Weekly and daily follow-up with to monitor infection(s) when needed.
- Administration of medications and vaccinations as requested by the physician or psychiatrist.
- Assess individuals upon admission and per state requirements thereafter meeting the needs of the individuals served.
- Under the direction of the DON, identifies, coordinates and monitors the health care of the individuals served, especially those with complex medical needs at assigned sites.
- Administration of first aid as needed.
- Provides care as ordered by physicians.
- Diabetic foot assessments.
- Assists with coordination of care and treatment for the individual served to ensure psychiatric, medical, dental, and optical appointments are scheduled as needed.
- Communicates and coordinates and physical health concerns and follow up treatment with physicians and psychiatrists.
