Hospice Sales Liaison

Nayar Hospice is expanding and seeking a high-performing, relationship-driven Hospice Sales Liaison to develop and grow our Richmond market. This is a growth-focused position responsible for building strong referral partnerships across hospitals, skilled nursing facilities, assisted living communities, physicians, and community organizations.

Key Responsibilities
- Develop and maintain referral relationships across all referral sources (hospitals, SNFs, ALFs, physicians, case managers, discharge planners, and community partners)
- Educate providers and families on hospice eligibility, services, and benefits
- Increase admissions and census growth within the assigned territory
- Track referral activity, follow-ups, and outcomes consistently
- Collaborate closely with Intake, Clinical Leadership, and Administration to ensure smooth transitions of care
- Maintain a consistent presence in facilities and community settings
- Meet and exceed monthly and quarterly census growth targets
Qualifications
- Minimum of 1 year hospice sales experience required
- Hospice or post-acute care sales experience strongly preferred
- Established referral relationships in Richmond
- Strong communication and presentation skills
- Self-motivated with proven ability to drive census growth
- Valid driver’s license and reliable transportation
