In this vital role, you will provide essential support to patients in their homes, promoting independence, comfort, and well-being. We are seeking caregivers who are faith-based, spiritually grounded, and respectful of our Christ-centered mission.

Responsibilities
- Assist patients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support using equipment such as Hoyer lifts.
- Monitor and accurately record vital signs, ensuring timely communication of any changes.
- Administer medications following prescribed protocols while maintaining proper documentation in EHR systems.
- Observe and report changes in patient condition, especially in cases involving dementia, Alzheimer’s disease, or memory impairment.
- Provide compassionate support to individuals receiving hospice care or living with physical or developmental disabilities.
- Prepare meals and assist with feeding while adhering to dietary requirements.
- Maintain a clean, safe, and peaceful home environment.
- Uphold HIPAA confidentiality standards at all times.
- Provide care in alignment with the agency’s Christ-centered values of compassion, integrity, patience, and service.
Requirements
- Valid New Jersey CHHA certification .
- Experience in home health care, long-term care, nursing homes, assisted living, or hospice preferred.
- Demonstrated ability to work with individuals with disabilities, dementia, Alzheimer’s disease, or in memory care settings.
- Familiarity with EMR/EHR documentation systems.
- Knowledge of medication administration, vital signs monitoring, catheter care, CPR certification, and hospice support.
- Strong observation skills, excellent communication, and compassionate bedside manner.
- Ability to work independently while maintaining professionalism and confidentiality.
- Must be faith-based and respectful of a Christ-centered work environment and mission.
Case Location - Rumson, NJ
Pay: $19.00 - $20.00 per hour
