Vitality Health Solutions is a growing, independently owned primary care practice seeking a compassionate and motivated Nurse Practitioner to join our team.
This is a 30-hour-per-week position with the opportunity to transition to full-time as your patient panel grows.We are looking for a provider who values patient-centered care, enjoys building long-term relationships with patients, and is comfortable practicing with a high level of autonomy.
Position Responsibilities
- Provide comprehensive primary care to adult patients
- Evaluate and treat acute and chronic medical conditions
- Perform routine physicals, preventive visits, and follow-up care
- Manage chronic conditions such as hypertension, diabetes, hyperlipidemia, thyroid disorders, and other common primary care conditions
- Order and interpret laboratory testing, imaging, and other diagnostic studies
- Prescribe and manage medications
- Provide preventive health counseling and appropriate screenings
- Coordinate referrals and collaborate with specialists and other healthcare professionals
- Maintain timely and accurate documentation in the electronic health record
- Develop strong, ongoing relationships with patients and participate in continuity of care
- Practice evidence-based medicine while providing compassionate, individualized care
Qualifications
- Current and unrestricted New Mexico Nurse Practitioner license
- National board certification as an FNP
- Current DEA registration or ability to obtain
- Current BLS certification
- Strong clinical judgment and ability to practice independently
- Excellent communication and interpersonal skills
- Comfortable managing both acute and chronic primary care conditions
- Primary care experience preferred; however, qualified candidates with strong clinical backgrounds are encouraged to apply
