Job Summary
PKHA is seeking a dedicated, culturally connected, and bilingual (English/Khmer) Community Health Worker (CHW) to join our team in Long Beach. In this role, you will act as a trusted bridge between the local Khmer senior community and essential healthcare delivery systems.
You will lead small-group health literacy classes, help low-income adults and seniors navigate their healthcare benefits, screen for social determinants of health, and coordinate care under California’s CalAIM / Enhanced Care Management (ECM) guidelines. This position requires deep empathy, community trust, and a passion for advancing health equity.
Essential Duties & Responsibilities
- Group Facilitation: Lead 2-hour, weekly in-person group health education and literacy classes in Khmer, focusing on chronic condition management, nutrition, and healthcare navigation.
- Care Navigation & Advocacy: Assist seniors with setting health goals, understanding medication instructions, translating health-related communications, and coordinating transportation or medical appointments.
- Digital Documentation: Accurately track participant attendance and complete brief, high-quality progress notes for each participant daily using our digital care tracking platform.
- Resource Coordination: Screen community members for health-related social needs (housing, food security, isolation) and link them to appropriate community-based programs and social services.
- Strict Regulatory Compliance: Maintain absolute boundaries between educational programming and insurance-related services, adhering strictly to HIPAA privacy regulations and California DHCS/CMS compliance guidelines.
Qualifications & Requirements
- Language: Must be fully fluent speaking both Khmer and English.
- Cultural Connection: Deep understanding of the Khmer community’s history, cultural nuances, and healthcare needs in the Long Beach area.
- Experience: Minimum 1–2 years of experience in community outreach, translation/interpreting, patient navigation, volunteering or social services. Experience working within a clinical setting or with Medi-Cal/CalAIM programs is a huge plus.
- Technical Literacy: Comfortable using tablets or laptops to navigate cloud-based portals, enter digital health notes, and manage calendar systems.
- Certifications:
- Current CPR / First Aid Certification (or willingness to complete a certified class within 30 days of hire).
- Completion of a state-recognized CHW Certificate program is highly preferred but not required.
