Job Summary
We are seeking a dedicated and compassionate Physical Therapist to join our healthcare team. The ideal candidate will have a strong background in physical therapy and a passion for helping patients achieve their rehabilitation goals. This role involves assessing patient needs, developing individualized care plans, and implementing therapeutic interventions to enhance mobility and quality of life. The Physical Therapist will work in various settings, including outpatient clinics, acute care facilities, and home community care environments.
Responsibilities
- Conduct comprehensive evaluations of patients to determine their physical therapy needs.
- Develop and implement personalized care plans based on assessment findings.
- Utilize various therapeutic techniques to improve patient mobility, strength, and overall function.
- Educate patients and their families on treatment plans, exercises, and self-care strategies.
- Maintain accurate documentation of patient progress and treatment outcomes.
- Collaborate with other healthcare professionals to ensure coordinated care for patients.
- Stay updated on the latest advancements in physical therapy practices and techniques.
- Provide support in acute care settings as well as outpatient facilities.
Experience
- A degree in Physical Therapy from an accredited program is required.
- Experience in geriatrics, orthopedics, or acute care settings is preferred.
- Proficiency in medical terminology and anatomy knowledge is essential.
- Familiarity with developing care plans tailored to individual patient needs.
- Previous experience in home community care or outpatient therapy is a plus.
- Strong communication skills to effectively interact with patients, families, and healthcare teams.
