Title: Social Worker

Salary: $70,000 to $75,000 Annually
Reports to: Senior Director of Congregate Housing
Status: Full-Time, Regular, Non-Exempt
Location: Mandaela Service Center (1074 Washington Ave, Bronx, NY 10456)
Days/Hours: Monday through Friday, 9am to 5pm
Position Summary:
The Social Worker provides ongoing clinical support to congregate housing participants, including individuals with histories of homelessness, justice involvement, mental health needs, and substance use challenges.
Working in collaboration with the Program Director and Case Managers, the Social Worker engages participants who require social service support and helps strengthen both program-based and individualized service delivery.
Essential Duties and Responsibilities:

Provide clinical support to the Program Director and Case Managers to help ensure enrollments/orientations are completed accurately and on time, including timely completion of psychosocial assessments;

Support Program Coordinators with group facilitation, participant engagement, and service coordination;

Provide training and guidance on trauma-informed practice and other factors that may affect participant engagement;

Assist the Senior Director with special projects related to ESSHI and DOHMH contracts and program requirements;

Provide clinical guidance to Case Managers in support of senior residents’ needs and service plans;

Deliver counseling and other clinical support services to participants, as needed;

Request and review supplemental information from referral sources, social service providers, and court advocates to support effective service planning;

Partner with Case Managers on complex cases requiring enhanced support, including coordination with ICM, CHS, and ACT providers;

Collaborate with Behavioral Health, SRP, and DOCS to ensure appropriate wraparound services are in place for participants;

Work with external providers to promote effective implementation of resources and services that support housing placement and stability;

Perform other duties as assigned.
Qualifications:

Master of Social Work (MSW) required; LMSW preferred;

At least one year of experience working with older adults experiencing homelessness is preferred;

Professional or internship experience supporting individuals affected by serious mental health conditions, trauma, or incarceration is preferred;

Bilingual Spanish-speaking ability is preferred;

Strong computer skills, including experience with electronic health records;

Strong participant engagement, counseling, and interpersonal skills;

Excellent clinical documentation, organizational, and time management skills;

A demonstrated commitment to the organization’s mission; relevant lived experience is valued.
