Position Summary
Ultra Care Home Health is seeking an experienced and compassionate Registered Nurse (RN) to join our team as a Salaried SOC Nurse. This position is primarily remote and field-based and is responsible for conducting Start of Care (SOC), Resumption of Care (ROC), Recertification, Significant Change, and Discharge visits throughout our service area.
The ideal candidate is organized, self-motivated, knowledgeable in OASIS documentation, and committed to providing exceptional patient care and customer service.
Responsibilities
- Complete Start of Care (SOC), Resumption of Care (ROC), Recertification, Significant Change, and Discharge assessments.
- Perform comprehensive nursing assessments and complete OASIS documentation accurately and timely.
- Develop and update individualized Plans of Care in collaboration with physicians.
- Coordinate care with physicians, therapists, office staff, patients, and caregivers.
- Maintain compliance with Medicare, Joint Commission, and agency requirements.
- Provide patient and caregiver education.
- Conduct follow-up calls with patients, caregivers, and/or Power of Attorney (POA) representatives throughout the episode of care.
- Address patient concerns and promote patient satisfaction through ongoing communication.
- Complete documentation within required agency timeframes.
QualificationsRequired
- Current Illinois Registered Nurse (RN) license in good standing.
- Minimum one (1) year of nursing experience.
- Current CPR certification.
- Valid driver's license, reliable transportation, and current automobile insurance.
- Ability to travel throughout the agency service area.
Preferred
- Home health experience.
- OASIS experience.
- Experience with Axxess or similar EMR systems.
- Knowledge of Medicare home health regulations.
Work Environment
- Remote and field-based position.
- Travel required for patient visits.
- Documentation completed remotely.
- Reports directly to the Director of Nursing (DON).
