Conduct comprehensive patient assessments and develop individualized care plans.

Administer medications and treatments as prescribed by physicians.

Participate in ongoing training and professional development.

Current LPN license in Massachusetts.

Proven experience in home health or longterm care settings.

Strong clinical assessment skills and knowledge of nursing best practices.

Excellent communication and interpersonal skills.

Ability to work independently and manage time effectively.

Familiarity with electronic medical record (EMR) systems is preferred.

CPR certification is required.

BLS certification is a plus.
