NewBridge Services, Inc. is seeking a Psychiatric APN licensed in NJ (Psychiatric / Mental Health) for a Part-Time position (15 hours/week) in a day treatment setting and outpatient counseling setting.
Summary: As an APN specializing in Psychiatric / Mental Health, you will be integral to delivering high-quality care. This part time licensed position provides comprehensive psychiatric and medication services to adults living independently with serious mental illness and co-occurring substance use disorders. The psychiatric services take place in both a day treatment setting and in an outpatient counseling setting where the APN will work with a team of mental health providers to ensure a team approach to treatment and recovery. Collaborative support and oversight is provided by the NewBridge Services Medical Director.
Responsibilities:
- Serve as a member of the mission driven, multi-disciplinary treatment team for the purpose of delivering outpatient services to adult mental health consumers through the use of evidence based individual and group psychotherapy and counseling; psychopharmacological treatment, medication management and monitoring, medication counseling; illness management and recovery education, family consultation and/or counseling. Each member of the treatment team is responsive to the needs of clients and each other and has responsibility for creating and maintaining a therapeutic environment that is safe.
- Enter into a collaborative agreement with a NewBridge Services Psychiatrist for ongoing consultation regarding scope of practice, psychiatric responsibilities, case reviews and services to be provided by the APN and Psychiatrist. This provides opportunity for both personal and professional growth and development.
- Provide staff with information regarding medications, side effects, expected treatment outcomes, and other medical issues related to medication management.
- Provide initial assessments, psychiatric evaluations, and psychiatric updates for clients assigned through the Access Department. The APN collects comprehensive health and mental health data that is pertinent to the individual’s situation; uses effective clinical interviewing skills; involves the clients, family and others in the client’s environment (as appropriate) in data collection; and synthesizes available data, including diagnostic tests, to determine diagnoses and level of risk.
- Develop an individualized treatment plan considering the client’s characteristics and situation in collaboration with the client. Includes strategies that address the identified diagnoses and desired outcomes.
- Implement and manage the treatment plan in a timely manner and according to regulatory and professional standards, documenting treatment plan reviews no less frequently than every 90 days during the first year of service and every 6 months thereafter.
- Use prescriptive authority, procedures, referrals, treatments and therapies according to state and federal laws and regulations. Provide clients with information about intended effects and potential adverse effects of proposed prescriptive therapies; prescribe pharmacologic agents based on a current knowledge of pharmacology and physiology, based on clinical indicators and results of diagnostic and laboratory tests; evaluate the effects and outcomes of treatments.
- Document consumer’s status and progress as related to evaluations, treatment, and community referrals, and according to regulatory and professional standards.
- Assist outpatient clients in obtaining access to medications through pharmaceutical assistance programs and other entitlements.
Experience: Minimum 2 years of experience with adults with serious mental illness and 1 year prescribing experience with psychiatric population. Computer literacy and ability to learn electronic health record software a must.
Schedule: 15 hours per week
Education: Masters of Science in Nursing
Certification:
- Advance Practice Nurse, state of NJ Board Certified RN.
- Psychiatric Mental Health Nurse Practitioner certificate.
