Home Health Intake Coordinator Position Summary
The Home Health Intake Coordinator is responsible for managing the intake and coordination of new home health patients from the time a referral is received through the start of care.
This is a one-person position, so the Intake Coordinator is responsible for staying organized, keeping up with referrals, scheduling, insurance verification, physician orders, and patient documentation.
The Intake Coordinator works independently but is also an important part of the office team. We are looking for someone who is dependable, organized, friendly, a good communicator, and a team player who can work well with everyone in the office.
Main ResponsibilitiesNew Patient Intake
- Receive and process new patient referrals.
- Enter new patient information accurately into Synergy, our EMR system.
- Review referrals and make sure all necessary information is received.
- Follow up on missing information or documents.
- Keep patient information organized and up to date.
- Follow each new patient from referral through Start of Care.
Insurance Verification
- Check patient insurance eligibility and benefits.
- Verify authorization requirements when needed.
- Enter insurance information into Synergy.
- Follow up on insurance or authorization issues that may delay services.
Scheduling & Assigning Staff
- Assign and schedule nurses for new and existing patients.
- Coordinate Physical Therapy, Occupational Therapy, Speech Therapy, Home Health Aides, and other ordered services.
- Provide nurses and therapists with the patient information they need.
- Communicate with clinicians regarding scheduling and patient assignments.
- Handle schedule changes and reassignments when needed.
- Make sure Start of Care and other important visits are scheduled in a timely manner.
Physician Orders & Follow-Up
- Send physician orders and other documents to doctors’ offices for signature.
- Keep track of outstanding orders.
- Follow up with physicians’ offices until needed orders and documents are received.
- Communicate with doctors’ offices regarding missing information, signatures, diagnoses, and other patient needs.
- Upload and file completed orders in Synergy.
Patient Records & Filing
- Scan, upload, and file patient documents.
- Keep patient records organized in Synergy.
- Check for missing documents, orders, or signatures.
- Make sure patient information is accurate and available to the appropriate staff.
- Keep up with filing and paperwork so outstanding items do not build up.
Communication
- Communicate professionally and respectfully with patients, families, nurses, therapists, physicians’ offices, and coworkers.
- Answer and route phone calls, emails, and faxes as needed.
- Keep nurses, therapists, and office staff informed of important patient updates.
- Follow up on outstanding patient matters in a timely manner.
- Bring clinical questions or concerns to the appropriate nurse or supervisor.
Teamwork
Although this is a one-person position, the Intake Coordinator is part of the overall home health team.
The Intake Coordinator should be able to work independently and take ownership of their responsibilities, while also working well with everyone in the office.

- Is a team player while still being able to work independently.
- Understands the importance of patient privacy and confidentiality.
Previous home health, medical office, intake, scheduling, or healthcare experience is preferred.
Overall Goal
The goal of the Home Health Intake Coordinator is to make sure new patients are processed correctly, staff are scheduled, orders are followed up on, records are organized, and communication is kept moving.
This person should be comfortable taking responsibility for the intake position while maintaining a positive working relationship with the entire office.
We want someone who can take ownership of their role, stay organized, communicate well, and be a dependable part of the team.
Pay: $25.00 - $30.00 per hour
