Responsibilities
- Assist clients with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, toileting, and mobility support
- Monitor and observe patients’ health status, including vital signs and overall condition, reporting any changes promptly
- Administer medications accurately following prescribed instructions while maintaining detailed medical documentation
- Support clients with specialized care needs including Alzheimer’s care, dementia care, hospice support, catheter care, and memory care
- Operate Electronic Health Record (EHR) and Electronic Medical Record (EMR) systems to document patient information securely and efficiently
- Use medical terminology appropriately when recording observations and communicating with healthcare teams
- Assist with light housekeeping tasks such as cleaning, laundry, meal preparation, and maintaining a safe environment in compliance with HIPAA regulations
- Utilize equipment like Hoyer lifts safely to assist with patient transfers and mobility
- Provide compassionate care for individuals with developmental disabilities or long-term care needs in home or assisted living settings
- Support patients’ emotional well-being through attentive listening and respectful interaction
Qualifications
- Proven experience working as a Home Health Aide or in similar caregiving roles within home health, nursing homes, assisted living facilities, or long-term care environments
- Strong knowledge of patient observation techniques and medical terminology
- Certification in CPR (Cardiopulmonary Resuscitation) required; additional certifications such as hospice care or dementia training preferred
- Experience with medication administration, catheter care, hospice support, and using assistive devices like Hoyer lifts is highly desirable
- Familiarity with EMR/EHR systems for accurate medical documentation and record keeping
- Ability to work independently while following detailed care plans and protocols
- Compassionate attitude combined with excellent communication skills to work effectively with patients and their families
