The Opportunity:

Responsibilities Include:

Making scheduled and as-needed visits to patient homes and facilities

Managing symptoms, monitoring health changes, and offering wound care

Supporting emotional needs and educating family members

Documenting clearly in our EMR system

Collaborating with a dedicated interdisciplinary team
What You’ll Need:
MN RN license in good standing

Experience in hospice preferred (palliative, oncology, or med-surg with EOL experience also welcome)

Confidence in working independently and managing time well

Strong interpersonal and documentation skills

Driver’s license and dependable transportation
