The Case Manager is responsible for supporting residents through comprehensive case management services, helping them navigate legal, personal, educational, employment, and community resource-related needs. This role serves as a key advocate for residents by coordinating care, maintaining compliance requirements, connecting residents to appropriate resources, and developing individualized plans that support long-term success and recovery.

Key Responsibilities
Schedule and conduct meetings with residents to assess needs and track progress.

Develop individualized treatment plans designed to support residents' personal, legal, educational, and employment goals.

Work collaboratively with care teams to ensure residents receive appropriate support and services.

Maintain accurate and timely case notes, documentation, and resident records.

Monitor resident progress and identify barriers to success.

Assist residents in accessing community resources, benefits, and support services.

Provide guidance and accountability to residents throughout their recovery journey.
Serve as a liaison between residents and external agencies, including probation officers, courts, attorneys, and other legal entities.

Monitor and support resident compliance with legal requirements.

Prepare and submit monthly program update letters and other required documentation.

Represent No Longer Bound in court proceedings when necessary.

Maintain detailed records regarding resident legal matters and case status updates.
Connect residents with community resources, educational opportunities, healthcare services, housing resources, and employment support.

Maintain current knowledge of local community resources, benefits, eligibility requirements, and referral processes.

Build and maintain relationships with community partners and service providers.

Advocate for residents while helping them develop independence and self-sufficiency.
Strong knowledge of community resources, government benefits, support programs, and eligibility requirements.

Ability to manage multiple cases simultaneously while maintaining accurate documentation and meeting deadlines.

Bachelor's degree in Social Work, Psychology, Counseling, Human Services, Healthcare Administration, or a related field with 1–2 years of relevant supervised experience.
Candidates without a bachelor's degree must possess a minimum of 4 years of direct case management, social services, behavioral health, recovery, healthcare, or related experience.
