Position Summary: The QIDP serves as the lead facilitator of the OISP in the residential team process for an assigned caseload.
Essential Functions:
- Functions as the Team leader, author and facilitator for assigned Ohio Individual Support Plans, following regulatory and agency requirements.
- Evaluates progress and makes appropriate revisions to OISP for assigned caseload of individuals.
- Works with core team to ensure individual needs are met.
- Completes all QIDP required regulatory, electronic and agency documentation.
- Provides input to the Team for Incident prevention plans and monitors for patterns and trends for Unusual and Major Unusual Incidents.
- Assists to ensure quality, compliant services and participates in plans of correction for regulatory reviews.
- Serves on internal and external committees.

Bona-fide Occupationally Required Competencies and Credentials:
- Bachelor’s Degree in education, psychology, human services, social services or nursing preferred. At least three years of paid, full-time or equivalent part-time experience in the field of developmental disabilities and in developing and/or implementing behavioral support and or/or risk reduction strategies, with at least one (1) year of supervision and management.
- Displays knowledge of Ohio Department of Job and Family Services, Ohio Department of Medicaid, Ohio Department of Health, Ohio Department of Developmental Disabilities and Center for Medicaid and Medicare Services rules and regulations.
- Must obtain and maintain certification in CPR & First Aid, Medication Administration and Health Related Activities and PMT.
- Must have basic computer and word processing skills.
- Must be able to communicate effectively both verbally and in writing.
- Must be able to follow verbal and written instructions.
- Must have a valid Ohio Driver’s License, good driving record and carry the proper liability insurance amounts required by The Society.

Travel: Meetings and site visits within the agency; meetings and seminars outside of the agency

FLSA status: Exempt, full-time (40 hours weekly with flexible scheduling)
Supervises: N/A
Works with: Individuals, guardians, families, external providers and professionals, medical specialists, nursing, department directors and administrative support staff, maintenance personnel, human resources and site management.

Physical demands: While performing duties of the job, employee is required to stand, walk, sit, drive, reach with hands and arms, talk and hear. Employee must occasionally lift and/or move up to 50 pounds. Work environment: The noise level in the work environment is usually moderate.
Based on results of the Hazard Assessment for Personal Protective Equipment (PPE), PPE may be required as outlined in the Infection Prevention and Control Manual and/or the Covid-19 Safety Preparedness and Response Plan. Required PPE may include: gloves, masks, gowns, goggles, face shields and/or N95 respirators.
