Summary: Responsible for overseeing and coordinating home health services for patients on the Leech Lake reservation. This role ensures delivery of high-quality, culturally respectful, patient centered care in the home setting. The manager supervises clinical staff, manages schedules and budgets while also working as a liaison in accessing other services our clients may need.
Education Requirements and Experience:
Current MN Registered Nurse license with at least two years of experience in Long Term Care facility, Primary Care or Home Care preferred.
Management experience preferred.
Additional Qualifications and Skills:
Excellent time management, organization, conflict resolution, public relations and communication skills are required.
Ability to establish and maintain effective working relationship with staff, patients, and other professionals
Assess, plan, implement, and evaluate individualized home health care plans.
All licensing renewal requirements as specified by the MN state Board of Nursing or as required by the state, including required CEU's.
Perform or delegate skilled nursing services, including wound care, medication management, chronic disease monitoring, and patient education.
Knowledge in writing policy and procedures within home healthcare.
Familiarity with the Leech Lake Reservation and surrounding communities is preferred.
Native American preference.
Knowledge of electronic health records (EHR).
Supervisory Duties:
Ensure all LLBO and state requirements for trainings, certifications and personnel files are kept up to date and current with reporting requirements.
There is expectation that the manager will expand services provided by this program and look at new grants and programs that can benefit the LLBO Home Healthcare program (HHC).
Supervise, mentor, and evaluate home health nursing staff.
Provide ongoing training with emphasis on cultural awareness and community-specific health needs.
Promote team collaboration and staff engagement.
Care coordination, facilitate referrals and ensure continuity of care across services.
Ensure adherence to federal, state, and tribal healthcare regulation.
Maintain accurate and timely clinical documentation.
Oversee quality assurance and performance improvement initiatives.
Manage program budgets, supplies, and resources efficiently.
Provide monthly reports to the Director of Nursing.
Visits patients in their homes, spends 25% of the position in homes.
Nursing assessments.
Provide individualized nursing care to patients.
Coordinate patient care.
Setting up medications, and evaluating patient's compliance.
Functions within the scope and standards of nursing practice as outlined in the Nurse Practice Act and Administrative Rules in state of practice and licensure.
Attend and or conduct all mandatory meetings and trainings as required.
Maintain confidentiality and professional ethics at all times.
Understanding and sensitivity to Native American culture.
Adhere to policies governing staff conduct.
All other related duties as assigned.

May require driving in sub-optimal weather and road conditions.

May require occasional modifications of workstations (this position will need to provide services in a large variety of settings and community locations).
Lifting requirement of at least 25lbs minimum.
