Minimum 2 years of recent Physical Therapy experience

Active PT license required at the time of consideration (license required if applicable)

No certifications expiring within the first 30 days of assignment unless documented renewal is scheduled

Valid U.S. Social Security Number required; Canadian SINs are not accepted

No gaps in employment of 90 or more days within the past 12 months

Up to 5 days of requested time off (RTO) is allowed but must be made up during the assignment

Candidates living within 50 miles of the facility are subject to a reduced bill rate and are limited to 3 consecutive 13-week contracts at this location; after completing 3 consecutive contracts, a minimum 1-year separation or transfer to a facility outside the 50-mile radius is required
