- Closely monitors status of all residents on assigned units.

Provides assistance with medication management, injections and treatments

Works with Director of Nursing to assure adequate staffing.

Assures all new residents are transitioned seamlessly into the facility.

Participate in the admission process including nursing assessments.

Completes NSA’s, full assessment and ongoing assessment.

Collaborates with treatment team for treatment plans and discharge for residents including with DSHS.

Create and update care plans.

Education/Experience

High School Diploma or a General Education Degree

LPN license through the Washington Department of Health (DOH) or Multistate Registry
Certificates/Training

Current CPR certificate

Our services include Short-Term Rehabilitation in our Health and Rehab facility, Enhanced Assisted Living, Adult Family Homes, Behavioral Health and Adult Residential Treatment in the Terrace facility, Memory Care, and Home Health.
