Harbor Care is seeking a SSVF Housing Case Manager to support Veterans experiencing or at risk of homelessness across New Hampshire. In this role, you’ll be at the forefront of life-changing work—helping individuals and families secure safe, stable housing and build a foundation for the future.

What You’ll Do

Provide strengths-based case management, including assessment, service planning, and ongoing support

Assist Veterans and their families with housing search, placement, and stabilization

Connect clients to critical community resources and supportive services

Monitor progress toward housing goals and adjust plans to ensure success

Maintain accurate documentation, including HMIS data and case records

Conduct outreach to identify and engage Veterans experiencing homelessness
You’ll also collaborate closely with internal teams and community partners—including the VA, Continuums of Care, and local providers—to ensure coordinated, effective care.
Required / Preferred Qualifications

Associate degree (Bachelor’s preferred) in Social Work, Psychology, Human Services, or related field
(or equivalent experience) -
Experience in case management, housing placement, or property management

Valid driver’s license and ability to travel as needed
Preferred Experience

Experience working with Veterans, individuals experiencing homelessness, or low-income populations

Veteran or Veteran family member status

Experience with documentation systems such as HMIS (or similar platforms)
