Overview:
We are seeking a reliable and personable Licensed Practical Nurse (LPN) or Registered Nurse (RN) to support and facilitate telehealth visits within Skilled Nursing Facilities (SNFs). This role is essential in ensuring smooth coordination between providers, patients, and facility staff, while maintaining a high standard of care and professionalism.
Key Responsibilities:
- Facilitate telehealth visits between providers and patients within SNFs
- Coordinate with facility staff to ensure timely and efficient patient encounters
- Prepare patients for virtual consultations (vitals, documentation readiness, etc.)
- Support providers during telehealth sessions and ensure proper follow-up
- Maintain accurate and timely documentation in accordance with company and facility standards
- Build and maintain positive working relationships with facility staff and patients
Qualifications:
- Active LPN or RN license (required)
- Strong communication and interpersonal skills
- Ability to work collaboratively with interdisciplinary teams
- Professional demeanor with a patient-first mindset
- Prior experience in skilled nursing facilities or telehealth is preferred
Requirements:
- Must be in good standing with all prior Skilled Nursing Facility employers
