1. Maintain monthly contact with participants to ensure that they are housed in safe, sanitary units; including working with participants in conjunction with assigned case manager to facilitate independent living as long as possible and working with the Client Placement Services Unit and the landlords to identify and access affordable housing resources.
2. Conduct intake interviews. Review all documentation to ensure completeness and compliance with CoC or HOPWA program eligibility requirements.
3. Set up and maintain individual participant files and ensure that all required forms are accurate, complete, current and audit ready. Work with participants to set up and maintain a budget, establish and/or repair credit. Track client progress and follow as indicated or assigned. Enter/retrieve data from program database.
4. Calculate rental payments, prepare lease/lease addendum in accordance with applicable agency and contract funder regulations. Work with the fiscal staff and/or landlords to resolve processing and payment problems.
5. Ensure landlord compliance with tenant/landlord laws and program criteria.
6. Maintain contact with participant’s assigned case manager. Notify case manager of case management issues. Identify problems and make referral(s) to ensure participants receive supportive services and/or other Pennsylvania Community Real Estate Corporation/Tenant Union Representative Network (PCRC/TURN) services for which they qualify.
7. Prepare and submit all required and/or assigned completed reports on time and accurately.
8. Maintain awareness of trends in housing, tenant/landlord issues and resources and treatment intervention for people with HIV/AIDS and other special needs population by participating in seminars, workshops, and conferences about housing and services for people with special needs. Work to educate the public about the program and its services.
9. Work with participants to maintain compliance with program requirements to remain a participant in the program.
10. Network with other social service providers and advocates to improve the quality and coordination of services for people with HIV/AIDS and other special needs populations.
11. Other duties as assigned.
