Loving Hearts Home Health Care LLC is seeking an experienced, dependable, and solutions-oriented Clinical Care Coordinator to support our clients, caregivers, and growing home care team.
This is not a traditional desk-only coordinator position.
We are looking for someone who enjoys working with people, communicates warmly and professionally, is comfortable using technology, and is willing to step into the field when needed. This position is ideal for an experienced caregiver, CNA, HHA, lead caregiver, or home care professional who is ready to grow into a leadership and coordination role while remaining connected to direct client care.
What You'll Do
As Clinical Care Coordinator, you will help create a positive and consistent experience for both our clients and caregivers.
Responsibilities may include:
- Conducting regular client and family check-ins
- Building strong relationships with clients, families, and caregivers
- Completing client welcome visits and quality-assurance visits
- Supporting caregiver orientation, mentoring, and field training
- Assisting with caregiver interviews and evaluating professionalism and fit
- Communicating with caregivers regarding schedules, client needs, concerns, and expectations
- Participate in community events associated with the Loving Hearts Home Health Care
Helping coordinate coverage when caregivers call off or shifts become unexpectedly open
- Providing occasional direct-care coverage when necessary to maintain continuity of care
- Identifying concerns and communicating/escalating them appropriately
- Documenting client and caregiver interactions accurately and promptly
- Collaborating with our remote operations team to ensure information and follow-up are properly documented
- Using company technology for scheduling, communication, documentation, forms, and other daily responsibilities
Traveling locally to client homes throughout our service area as needed

- Still comfortable providing hands-on care when occasional coverage is needed
You do not need to know every technology platform we use before starting. You must, however, be willing and able to learn and consistently use required technology.
Preferred Background
- CNA, HHA, or experienced professional caregiver
- Previous home care or home health experience
- Lead caregiver, field supervisor, scheduler, care coordinator, or similar experience is a plus
- Experience with dementia, personal care, and older adults preferred
- CPR/First Aid certification preferred
- Valid driver's license and reliable transportation
- Ability to travel to client homes throughout the service area
- Smartphone and ability to use required mobile applications
Schedule
Part-time
Approximately 20–25 hours per week
- Hybrid position combining remote administrative responsibilities and local field/client visits
- Some flexibility in scheduling is required based on client and caregiver needs
- Occasional direct-care coverage may be required when necessary
This is not a full-time direct-care position. The coordinator's primary responsibility is supporting clients, caregivers, quality, communication, and continuity of care.

Pay: $25.00 - $28.00 per hour
