Pathways to Wellness Medication Clinic is an outpatient mental health treatment program founded in 2000 to meet a significant need within our mental health community. Pathways to Wellness is designed to provide a vital link in the spectrum of services available for adults requiring mental health management. Recently opened up services to children and adolescents. Staffed by Board Certified Psychiatrists, Nurse Practitioners, Physician Assistants, Clinical Pharmacists, Psychiatric Nurses, and Licensed Clinicians, our clinics provide services that are intended to afford significant relief from the symptoms of mental illness, to facilitate our patient’s ability to return or remain in the community, and to avoid treatment at higher levels of care. Pathway to Wellness company seeks a full-time Psychiatric Registered Nurse for our outpatient mental health clinic located in Pleasanton. [ask our recruiter about opportunities for full-time employment]
Full-Time: 40hrs per week
Clinic Hours: Monday - Friday 8:30 am - 5:00 pm
GENERAL RESPONSIBILITIES: Psychiatric Program Nurse will work in a very fast past high volume outpatient medication clinic setting as a member of an interdisciplinary team, in a community-based system of care for clients with serious mental illness and co-occurring disorders. The nurse will complete vitals, psychiatric assessment, medication checks & medication refills, treatment plans, injections, safety plans, clinical case management services, coordinate labs, complete medication authorizations, run CURES reports, and complete other nursing tasks as requested. The nurse will work with acute hospitals and community resources to coordinate clients' clinical care and recovery services. Must be able to work in a very fast pace work environment. QUALIFICATIONS
- 3 years experience working in inpatient or outpatient mental health
- California license as a Registered Nurse.
-Master's degree in Psychiatric Nursing is preferred but not required.
-BSN Certification as a Psychiatric/Mental Health Nurse is preferred but not required.
REQUIRED SKILLS:
Must be organized, task-oriented, reliable, flexible, possess strong technological skills , excel , and committed to working with outpatient mental health clients. Understanding of Medi-cal/Medicare documentation requirements and use of Electronic Health Records. Provide daily medication checks and injections.
SPECIFIC RESPONSIBILITIES:
- Assist treating medical providers with medication checks.
-Devise nursing care plans, which include patient referral or admission, medication doses, monthly medications checks with a physician, taking vitals each visit to the clinic, administering therapies and treatments, evaluation of results, and summation of the plan's effectiveness.
-Provides ongoing communication and collaboration with onsite medical Providers to ensure well-coordinated care within the program.
-Provides ongoing communication and collaboration with identified Primary Care Physicians and/or each patient's Medical Care Home personnel to ensure proper discharge planning and transitioning from Pathways to Wellness care.
-Act as liaison between patients, their families, and health care personnel to ensure efficient and prompt health care service.
-Completes required assessments on all new admissions.
-Daily documentation and participation in the implementation of treatment plans for clients.
-Notes physician orders and follows through as needed.
-Assists with maintaining and monitoring the accuracy of patient medical records.
-Weekly follow-up and telephone outreach with patients who end up in higher levels of care [i.e. Emergency room, psych hospital, jail, etc.]
-Participates in Provider/Staff meetings, Treatment Team meetings, and Continuous Quality Review meetings.
-Coordinates all laboratory procedures, including Infection Control responsibilities.
-Assists with coordinating patients' scheduled appointments/sessions with medical staff.
-Conducts patient medication education groups and provides individual client education when necessary.
-Other duties as assigned
